Office of the University Registrar Forms
Important University-wide forms are routed to the School of Engineering for approval. Please work with your advisor when submitting any of these, or if you have questions about the forms. Commonly-used forms are available below. Specific instructions for completing these are found on the Schedule Changes Forms of the Office of the University Registrar.
If you are requesting a Petition to Late Enroll, please see your advisor and work with them on submitting the petition. This is available online and on-paper. The Office of the University Registrar prefers the paper form for the Petition to Late Enroll to quickly enroll students. More information on late enrollment is available at Schedule Changes Forms, under the Late Enrollment heading on the University Registrar website.
- Request to Enroll After the Deadline to Add Online. This form can handle late additions and the maximum hours approvals after the 5th day of classes of the semester.
- Time Conflict Form
- Request Permission to Enroll Above the Maximum Credit Hour Limit. This form is used prior to the 5th day of classes of the semester.
- Request to Adjust Credit Hours Form. This form is only needed after the 5th day of classes of the semester.
